Below are new patient forms. Please complete the New Patient Appointment Request and give it to our receptionist before scheduling your first visit.

You should read the Notice of Privacy Policies before your first visit, and will be asked to sign an acknowledgement that you read it. You should also read the Welcome to the Practice letter. It will tell you about our office hours, location, prescription refill policy, and other helpful information.

Office Policies

Prescription refills

Please allow 48 hours for prescription refills. It is best to contact your pharmacy and request a refill that they will send electronically to us. We cannot refill prescriptions after hours or on weekends.

After hours illness

In an emergency always dial 911. If you are sick after office hours or on weekends, you may need to go to an urgent care or the hospital Emergency Room for evaluation. Otherwise, call the office during regular office hours and we can get you in that day or the next.  Members have the doctor’s phone number on their MDVIP card if they need to contact them after hours.

    Dr. Steve and Dr. Stephanie only see patients enrolled in the MDVIP Wellness Program.
